Community Development and Health Promotion (CD/HP) staff can work together and in collaboration with individuals, families and caregivers to support the referral process for accessing services and supports available to them in their communities, such as housing, food access, home care, mental health and addictions support, seniors safety, youth services, seniors services, women’s services and more.
