Primary Health Care

The Primary Health Care Program consists of a team of physicians, nurse practitioners, nurses and other allied health professionals (social worker, dietitian, physiotherapist) who work in collaboration with other programs offered by Brock CHC.  Services include:  preventative health care, treatment of chronic and episodic illnesses, mental health counselling, service navigation and referrals, access to phlebotomy, and more.  All Brock CHC services are free.

Brock CHC is currently not accepting new Primary Care clients.

Brock CHC’s staff are facing high patient workloads while we recruit for additional providers.  At this time, without additional funding, we are unable to accept new clients.

For those in the community that do not have a family physician, we are keeping a waiting list.   Our current wait time is 2 years. If you would like to be added to our wait list, please complete the Request for a Primary Health Care Provider form which can be picked up in person, mailed, or downloaded.

To help you find a family physician or nurse practitioner, you can register with Health Care Connect.
