Brock CHC Receives $1.6 Million to Expand Primary Care Services in North Durham

Brock Township, ON – Brock Community Health Centre (Brock CHC) is proud to announce it has been selected by Ontario Health to receive $1.6 million in funding through the Expanding and Enhancing Interprofessional Primary Care Teams initiative.

This significant investment will enable Brock CHC to expand its existing interprofessional primary care team and increase patient access and attachment to comprehensive, team-based care across Brock Township and surrounding areas.

With this funding, Brock CHC will welcome a range of added healthcare professionals, including an additional family physician and nurse practitioner, registered practical nurses, a social worker, and a dietitian, as well as a system navigator. These additions will strengthen the Centre’s ability to provide coordinated, patient-centered care that addresses the diverse needs of the community.

“We are very excited about this announcement.  This investment represents a transformative step forward for our community,” said Janet McPherson, Executive Director. “By expanding our interprofessional primary care teams, we can improve patient attachment and access to timely care, enhance patient outcomes, and better support the health and well-being of local residents in our rural community.”

The expanded team will support the collaborative development of a North Durham Health Home with Brock Community Health Centre, North Durham Family Health Team, Medical Associates of Port Perry, Uxbridge Health Centre and the Toronto Street Medical Centre, connecting individuals and families to a wide range of primary care and community-based services close to home.

This funding aligns with Brock CHC’s ongoing commitment to improving access to care, reducing wait times, and addressing the growing demand for primary care services in rural and underserved communities.
