Brock Community Health Centre (Brock CHC) is a new non-profit organization committed to providing quality primary health care and related services to the residents of Brock Township and surrounding area. We are governed by a Board of Directors consisting of community leaders, professionals and other representatives from Brock Township and vicinity. Funding is provided by the Ontario Ministry of Health and Long-Term Care (MOHLTC) and the Central East Local Health Integration Network (CE LHIN).
A range of community-based health services are provided by an inter-professional team including physicians, nurse practitioners, registered nurses, registered dietitians, health promoter, community health workers, social worker, and other health professionals needed in our community.

Services are provided by a collaborating inter-professional team supported by committed community partners.
