Brock Geriatric Assessment Clinic
The Brock Geriatric Assessment Clinic (BGAC), an innovative partnership between Brock CHC, Lakeridge Health, Durham Region and the Regional Geriatric Program of Toronto, commenced operation in 2009. It provides comprehensive geriatric assessment for frail, elderly residents of Brock Township and vicinity. State-of-the-art equipment and assessment capabilities are available. The program uses a distributed professional team model including an on-site Primary Care Nurse Practitioner who accesses geriatric consultation services and expertise through the use of video conferencing technology. Access to these services is through self-referral, referral from a family member or referral from a family physician.
